To expedite your enrollment, please be sure to have the following items:

Your personal information - Social Security Number, home ZIP Code, Date of Birth - as well as the information for your spouse and/or dependents

Group number - this will be listed in the email instructions we sent to you

Provider ID - to take advantage of online tools such as The Plan Comparison Tool, Provider Search, and Rx Search.

Register and Create Your Account
Set up your CaliforniaChoice username and password.
Verify Your Personal Information
Confirm your Social Security Number, quote number, and home ZIP Code. If applicable, you will be asked to include information for your spouse and/or dependents.
Add coverage for your Spouse and/or Dependents
If applicable, provide personal information and select coverage for your spouse and/or dependents.
Select Your Plans
Review the options available to you and select the plan that fits your needs.
Review and Submit
Review your enrollment summary carefully, make any necessary changes, and submit your application.
