Four Dental HMO (DHMO) options are offered through CaliforniaChoice: the SmileSaver Plan 3000 and Plan 1000 and the MetLife MET100 and MET185 plans.

 

All DHMO plans offer low or no-cost services for the following:

• Exams and diagnostics
• Office visits
• Initial oral exam
• Periodic oral exam
• Teeth cleaning
• X-rays (bite-wing, 4 films)
• Restorative
• Cavities (amalgam, 1 surface)
• Cavities (amalgam, 2 surfaces)
• Periodontics
• Gingivectomy – per tooth
• Periodontal scaling and foot planing (quadrant)

 

Plans also include benefits for Oral Surgery, Endodontics, Crowns, Orthodontics, and Prosthodontics. All services are subject to certain exclusions and limitations, which are detailed in the enrollment materials.

The MetLife DHMO plans provide access to a large network of dental providers throughout California. Preventive dental care is an important part of overall health and MetLife is committed to ensuring that its members receive a high level of dental care. As part of this commitment, all Quality Management Activities are designed to meet or exceed NCQA standards. These standards are applied to plan design, through the credentialing of network providers, member services standards and on-going peer review and facility audits.
